A sports history and exam with our Nurse Practitioner help young athletes determine if it’s safe to participate in sports.

Annual Wellness Exams (Men and Women)

Men and Women: Stay up-to-date on important health screening changes and illness prevention with an annual physical exam with a holistic wellness approach. This includes screening for cancers with PAP test, breast exams, an order for mammogram, referral for colon cancer screening screening, and genetic testing.

STI Screening

Screening for STIs is recommended by the CDC annual and with any new sexual partner, regardless of symptoms of infection.

Preconception Evaluation

A wellness evaluation with education for women and men anticipating a pregnancy.

Body Literacy for Teens

The puberty education you wish you had! Body Literacy education empowers young people (11-17) to understand the holistic basics of puberty, interpret hormone changes in their bodies, and make decisions for greatest health outcomes in the future.

Cycle Syncing for Health and Wellness

Cycle Syncing empowers women to confidently identify signs of hormonal balance, equipping them to understand their cycle in a state of health and harness the holistic benefits of healthy ovulation.

Kohl and his Generation Z peers are ready for the life-changing impacts of the Connect Experience.

At Connect Medical Clinic, prevention, balance, and wellness are at the heart of what we do—and young people like Kohl truly embody that spirit. Introduced by his mom, Kohl discovered firsthand the transformative power of dignity-centered care. As an avid football and hockey player, his journey began with a routine sports physical. “The staff explained everything, asked if I was comfortable, and made me feel like I wasn’t just a number,” he recalls.

Realizing the importance of a safe, holistic approach to health, Kohl became a passionate advocate for his peers. “Kids my age have extracurriculars that come with risks. It’s important to have a safe place to go where people truly care and want us to succeed,” he explains. Motivated by his own experience, he connected his teammates, classmates, and even school resource officers with Connect. This commitment to wellness and prevention was celebrated when the clinic provided his team with a PayDay Bar after a game, along with a heartfelt reminder: “You are Priceless.”

Reflecting on the broader impact, Kohl shares, “Once I got to thinking about how my kids can be affected by the things that happen to me now, or in my generation now, I knew I wanted to keep my teammates safe, and everyone at my school, to have a healthy future community.” Inspired by his mother’s genuine care, he adds, “She truly cares about people, a lot. I probably need to work on that, genuinely caring about others.”

Connection Services are tailored for individuals of all ages—from young athletes and teens to adults—who seek proactive wellness, preventive care, and holistic education to foster natural function and healthy relationships. Our compassionate providers empower you to understand and connect with your body, supporting informed decisions that pave the way for a vibrant, hopeful future.

Sports Physicals

Our providers offer thorough and compassionate sports physicals to ensure your child is healthy and ready for the season ahead. During the appointment, they will review medical history, check vital signs, assess physical fitness, and screen for any underlying concerns that could impact athletic performance. It’s an excellent opportunity to ask questions and make sure your athlete is set up for a safe, successful season—with care that’s always supportive, respectful, and family-centered. Just $25.00!

Annual Wellness Exams (Men and Women)

Our providers deliver personalized, dignity-centered annual exams focused on prevention and early detection. Each visit includes a review of your medical and family history, vital signs, lifestyle habits, and gender and age-appropriate screenings. Depending on your needs, this may include breast, pelvic, testicular, or prostate exams, screenings for cancers like colon, cervical, or skin, as well as assess your genetic risk for certain types of cancer to guide personalized prevention and early detection strategies. Lab work, and referrals for immunizations and further care may also be included.

STI Screening

The CDC recommends an STI screening evaluation annually and with any new sexual partner. Screening refers to a routine test, performed regardless of signs or symptoms of infection. STI screening evaluations consider the patient holistically, with a conversation that helps tailor the screening to your needs in a completely judgment-free and respectful environment. Testing may include a urine sample, blood test, swab from the throat, genitals, or rectum, with some of these being collected privately by the patient in the bathroom. A genital exam may be performed if indicated. If anything comes back positive, we’ll support you with treatment and next steps.

Preconception Evaluation

This holistic evaluation is recommended for both partners three months ahead of trying to conceive to prepare to ensure health and lifestyle are optimal for a healthy pregnancy. Includes a wellness-reproductive exam; may include lab tests, including genetic testing for both men and women. Separate or individual appointments. We are here to help you with a life-affirming, holistic approach to achieving pregnancy.

Body Literacy for Teens

Body literacy for teens refers to understanding and being aware of how your body works, especially when it comes to reproductive health, physical changes, and emotional well-being. It involves learning about your body’s natural rhythms, like menstruation, hormones, and how lifestyle choices can impact your health. Body literacy encourages teens to track and understand their physical and emotional patterns, recognize the signals their bodies give them, and feel empowered to make informed decisions about their health. It’s about cultivating respect for your body, understanding its needs, and having the confidence to advocate for your well-being.

Cycle Syncing for Health and Wellness

Did you know ovulation is a fifth vital sign for women? Cycle Syncing education can help women understand their cycle in a state of health and harness the holistic benefits of a healthy ovulation. By understanding the hormonal fluctuations during each phase, cycle syncing helps optimize physical, mental, and emotional well-being throughout the month. Involves cycle charting education and evaluation of chart with our provider for health recommendations. *Our team are FEMM Instructors; see femmhealth.org for more details. This education service includes 4 sessions with our medical care team.

ConnectED

A personalized, holistic education and support program designed for all of our patients. ConnectED goes beyond traditional care by offering tailored guidance on various health and life topics. With each in-person lesson review with our medical care team, patients earn practical items that support their journey. Our focus is on building strong, supportive relationships, equipping a person with the tools and knowledge to support their wellbeing. It’s more than just education; it’s a pathway to empowerment.

Our consultation is an appointment to be done prior to any other scheduled appointments where we assess your greatest needs through a detailed questionnaire. We also do an initial review of your demographic information, allergies, and medications. We will finish up the appointment by talking through our payment options. We accept insurance: BCBS, Sanford, United, and Medicaid. We also have a self pay option if you are without insurance, have a plan we do not accept, or would prefer not to use it. Our self pay pathway operates on a generous sliding scale based on household income.
